Here is the very crux of today’s problem. How many errors or omissions, by negligence or intent, have influenced the data’s analysis on the way to the top? How many of those decisions, based on that data and its analysis, that need to be made per day, per hour, per minute and eventually per second are wrong and begin to erode the function of the entity?
